ATHLETIC CLEARANCE

All students must be cleared through our ONLINE athletic clearance process BEFORE the first tryout date of a tryout sport, or first practice date of a non-tryout sport. It is recommended you have your clearance completed at least one week before. 

STEP 1: ONLINE CLEARANCE - www.athleticclearance.com

Log-in Instructions - Please note that students participating in multiple sports will only need to be cleared once but will need to add each sport individually. Our athletic director will review your online clearance and mark the student cleared when all steps have been completed. You will receive an email when cleared.

     ATHLETIC PHYSICALS MUST BE DATED JUNE 1, 2024 OR LATER FOR THE CURRENT SCHOOL YEAR

 

STEP 2: SPORTS PHYSICAL

Sports Physical - A new sports physical is required for each school year and will need to be uploaded to the online clearance site linked above. A sports physical is valid with the following:

• Part 1 student information completed
• Part 2 health information questions answered
• Parent information completed with signature (this step is frequently missed)
• dated after June 1st of the current school year
• signed by a physician (M.D., D.O., P.A., or N.P. only)
• stamped by a physician
